Most adolescents never start out thinking they are going to get over their head with substance abuse. When they do, the Transitions program can offer outpatient substance abuse counseling and treatment that specializes in services to adolescents. By using standardized assessment tools to identify the needs of the adolescent, we can provide the best possible therapy and setting. Substance Abuse Counseling is available to adults as well.

Transitions provides quality, effective and timely services in an effort to:

• Limit the negative consequences of substance abuse
• Increase the likelihood of successful recovery and,
• Assist in the prevention of a relapse

Services include Assessment, Evaluation/Referral, Individual/Family Counseling, Group Counseling and Continuing Care/Relapse Prevention.

There is a per-session fee charged for counseling groups and individual services. Insurance and Medicaid is accepted. Please see our brochure, TFI Family Services Substance Abuse Program, for more information.
